任何想微整或手術的求美者應該要了解，美容手術追求的是改善而非追求完美，林醫師在諮詢時不愛強調效果，偶爾會潑一點冷水，以隆鼻手術來說，本來軟骨支撐弱且扁平的鼻頭，術後要變成西方人的高挺鼻是不容易的，就像地基不穩是無法蓋高樓的。

此案例是典型的東方人鼻頭，支撐較弱，且受限於本來的結構會在墊高時產生反作用力，即使使用很強的軟骨材料，鼻頭尖挺度改善也有限，在此狀況下如何創造出符合美學比例的曲線就很重要。

案例多年前在其他診所接受矽膠隆鼻後穿出感染，鼻頭組織攣縮而且纖維化，鼻尖處有一明顯的凹陷，使用肋軟骨重建鼻頭，讓她恢復自然立體的鼻尖。

A beautiful lady had a silicone implant augmentation rhinoplasty years ago suffered from implant extrusion and infection. She came to me seeking for a revision. After debridement and nasal tip reconstruction with costal cartilage, we created a nasal tip to achieve both a more balanced profile and also a nasal tip that looks more refined with better definition from the lateral and the front.

將十年前的矽膠鼻模取出後，用耳軟骨及Goretex創造出微翹鼻頭

She had a silicone nasal implant 10 years ago. She came to me seeking a more refined nasal tip. I replaced the previous implant with ePTFE implant and used auricular cartilage for septal extension and cap graft to increase her tip projection and nasal length.

使用耳軟骨雕塑鼻頭及Gore-Tex增高山根，改善扁平的山根及較不立體的鼻頭，術後約一個半月，還沒完全消腫，不過已經呈現自然的外觀。

I used autologous auricular cartilage for tip projection and down rotation, ePTEF for nasal bridge augmentation. It’s post-op one and half months, still far from final shape, but even with the swelling you can see the increased definition and refinement of the nasal tip, the greater height and length of her nose, and a natural result she is looking for.

案例曾在其他診所做過隆鼻，後來經歷了假體穿出感染，導致鼻頭軟組織缺損，鼻頭凹陷，經過鼻頭軟骨重建手術，鼻頭恢復了自然和立體感。

駝峰鼻是發生在鼻背軟硬骨的交界處，因為軟骨硬骨的過度發育，導致鼻背凸起，有時會合併有鷹勾鼻，林醫師除了將駝峰移除，也矯正了鼻中膈彎曲造成的鼻塞，並調整了鼻頭形狀，傷口在鼻孔內，術後看不見疤痕。

案例的鼻子缺乏山根，鼻頭較圓潤，鼻頭的皮膚也很厚，讓五官也顯得不立體，如果要創造出立體精緻的鼻頭，需要強而有力的軟骨結構支撐，因此建議使用肋骨隆鼻。

林醫師將她的鼻頭結構重新調整並撐高，用Gortex墊山根，經過鼻整形手術後，將鼻子的曲線和豐潤的額頭連接得恰到好處。
